Chronic lower back, hip, or leg pain can be the result of spasmed or shortened hip flexor muscles which consist of the psoas and iliacus muscles. The psoas originates from the lower lumbar vertebra and the Iliacus originates from the inside and top of the pelvic bone, they combine to
form the iliopsoas muscle and attach to the inner femur (thigh bone). If the iliopsoas muscle is overstretched from activities such as running or jumping ,or weekend from prolonged sitting, it can spasm causing a pulling on the lumbar vertebra, this tension can lead to a increased lumbar curve (swayback), which then causes chronic low back pain. The constant muscle spasm then can cause trigger points to form in the muscle tissue, these trigger points may not be painful unless they are pressed, or they can cause hip, groin, or thigh pain. The pain is generally of a constant dull ache in the low back, which may have some accompanying pain in the front of the hip or thigh, it is usually difficult and painful getting up after sitting and it may take some time to straighten up. Flexing your knee up to your chest is generally painful, and standing for long periods also aggravates the pain. Sitting generally helps relieve the pain. The tight muscle can also cause a popping sound when the hip is flexed due to the tendon catching on a bony structure in the pelvis (snapping hip, see below).Most often the popping does not cause pain, but it can be indicative of a shortened iliopsoas muscle, and a painful low back or hip condition may occur later. When you have chronic low back pain that does not respond to regular low back treatment, or continually returns, the hip flexors should be evaluated and treated if necessary.
